Private Care Management & RN Advocacy
Comprehensive Oversight.
Thoughtful Coordination.
Relentless Advocacy.
When an aging loved one's needs become complex, families are left managing multiple providers, conflicting information, and high-stakes decisions, often from a distance, and always under pressure.
You don't have to navigate it alone. As your family's private RN advocate and Certified Aging Life Care Manager®, I help families navigate complex healthcare with confidence by coordinating care, guiding important decisions, and ensuring nothing important is overlooked.
What Is Aging Life Care™ Management?
An Aging Life Care Manager® helps families navigate the many decisions that come with aging, illness, and long term care. From coordinating medical care to helping families understand their options, an Aging Life Care Manager brings clarity, organization, and experienced guidance during times that often feel overwhelming. As an Advanced Professional member of the Aging Life Care Association®, I practice across the profession's eight knowledge areas:
Health & Disability
Attending appointments, facilitating physician-family communication, and engaging and monitoring the right services, from home health to hospice.
Financial
Coordinating with accountants, Powers of Attorney, and advisors; guidance on entitlements and insurance concerns.
Housing
Evaluating and selecting the appropriate level of residential care or in-home support.
Families
Helping families cope, problem-solve, and align around long-term care decisions, including long-distance caregiving.
Local Resources
Knowing which services exist in the community and how to access them.
Advocacy
Promoting the client's wishes with providers and ensuring needs are truly met.
Legal
Connecting families to elder law attorneys, estate planners, and fiduciaries; supporting court-related care determinations.
Crisis Intervention
Navigating emergency departments, hospitalizations, and rehabilitation stays, while providing guidance and support during critical transitions.
What sets our practice apart?
We bring these eight areas together with the clinical judgment of a Registered Nurse, a depth of medical insight most care management practices cannot offer.
The RNCC Difference
A higher standard of care and a more personal level of service.
As the founder of RN Care Consultants, I bring a background in critical care and years of leading healthcare organizations, giving our practice a deep understanding of both the medical and practical aspects of care. We help families navigate complex healthcare situations with confidence while coordinating every detail with compassion and professionalism.
We intentionally work with a limited number of families so each client receives the time, attention, and continuity they deserve.
Specialized Support
Long-Distance Caregiving Support
Local eyes, ears, and advocacy for families caring for an aging loved one from out of town. We provide regular updates and oversight so you're never left wondering what's happening on the ground.
Hospital Discharge & Care Transition Planning
The days after a hospitalization or rehabilitation stay are when things most often fall through the cracks. We coordinate every detail of a safe transition home, from discharge instructions to in-home support.

Guidance, education, and steady support for individuals and families navigating dementia and cognitive decline, from early changes through long-term care decisions.

Frequently Asked Questions
How is a care manager different from home care?
1
Home care provides hands-on assistance with daily activities such as bathing, dressing, meal preparation, and companionship. A care manager oversees the bigger picture by coordinating medical care, advocating for your loved one, communicating with healthcare providers, and helping your family make informed decisions.
Can you attend medical appointments with my loved one?
2
Yes. When appropriate, we can attend medical appointments, help explain diagnoses and treatment options, ask important questions, and ensure your family's concerns are addressed. Our goal is to help you make informed decisions with confidence.
Do you work with families who live outside Los Angeles?
3
Yes. We work with families throughout Los Angeles and also provide consultations for select clients nationwide. Many of the families we support are coordinating care for a loved one from another city or state.
When is the right time to hire a care manager?
4
The best time is before a situation becomes overwhelming. Whether you're planning ahead, facing a new diagnosis, recovering from a hospitalization, or managing ongoing care, early guidance can help prevent unnecessary stress and ensure the right support is in place.
Do I need a care manager if my loved one already has a caregiver?
5
Yes. Caregivers provide day-to-day support, while a care manager oversees the overall care plan. We help coordinate healthcare providers, monitor changing needs, communicate with your family, and ensure every aspect of care is working together.
How often will you see my loved one?
6
Visit frequency is tailored to each client's needs. Most clients are seen weekly or biweekly, though some situations call for more frequent involvement, such as during a hospitalization, a transition home, or a change in health. We regularly reassess each care plan and adjust the level of support as your loved one's needs evolve.
